Club Inventory Report Form - Use this form to record items collectively owned by a club or organization. This must include all items, not just items recently purchased or purchased this year. All items purchased with SGA money (including money allocated by all funds, your budget, the Educational Council, etc.) must be reported. This document must be submitted alongside all club and organizations’ audit reports.
Club Registration Form - This form must be submitted at the beginning of the academic year, with audits, and with budget proposals. Please include only members who come to meetings; it is not permissible to put on the form people who once showed interest in your club and never came to meetings. Please note that this year’s Club Registration Form includes a separate section for members of clubs’ executive boards.
Club Budget Ledger Template - This is a template budget ledger. If you are familiar with Excel, you can use this pre-formatted spreadsheet to record your transactions. If you are unfamiliar with Excel, you can print out this document and record your transactions by hand. It is recommended that you view the budget ledger example below.

Programming Evaluation Form - This form must be submitted alongside all club and organizations’ audit reports. This form must include evaluations for all programs that were run by your club/organization. Information that is provided in this form will be considered in fund allocation for the following fiscal year.
